2021-2022 Kindergarten Registration and Screening Information

KG Registration

***If you have registered your son or daughter for Kindergarten this year, you should receive a phone call from your child's building secretary to set up a time to return your information sheets during the week of March 15th.  Look for Registration Information delivered to your address which you provided in the registration form ***

The information below explains the kindergarten registration and screening process at all River View Elementary buildings.  If you have any questions, please contact the office at the school your child will be attending.

Follow these four steps to register your child:

Step 1: Please complete the online registration form.

Conesville Elementary Registration Form
Keene Elementary Registration Form
Warsaw Elementary Registration Form

Step 2: Once you have completed the form, watch your USPS mail after March 15th, for the registration packet.  You do NOT need to print anything from this page.

Step 3:  You will receive a call from the school secretary during the week of March 15th.  Please follow the instructions given.

You must take the REQUIRED documents listed below when you register your child during the time you set up with the building secretary.

Required Documents for School Entrance:

BRING ALL of these to the Kindergarten Registration Meeting

We will make copies and return them to you.

  • Custodial Papers (if applicable)
  • Legal Birth Certificate (student must be at least 5 on or before August 1, 2020) This needs to be the birth certificate issued by the health department.  Certificates of Birth issued by the hospital at the time of birth are not legal documents.  A legal birth certificate can be obtained at any local health department or from the State of Ohio (or state of birth) by mail.  Forms are available at the school or by visiting the Ohio Department of Health website at or by calling 614-466-2531.
  • Immunization Record

All children entering kindergarten must have the following immunizations completed before they will be permitted to begin school in the State of Ohio:

  • 5 doses of DPT (diphtheria/tetanus/pertussis)
  • 4 doses of Polio
  • 2 doses of MMR (measles/mumps/rubella)
  • 3 doses of Hepatitis B
  • 2 doses of Varicella (Chickenpox Vaccine) or must have had actual chickenpox disease.

Immunizations can be obtained anytime between now and the first day of school. It is highly recommended that you do not wait until August to have your child's vaccines updated, as appointment times are limited.  Your child may be excluded from school if immunizations are not completed by Sept. 11, 2021.

Please have your doctor or an agency provide you with a new copy of the updated record when the immunizations are given and FAX or email the updated record to the School Nurse at the address below.

If you have any questions, please feel free to contact your physician or Kayla Davis, RN, District Nurse at 824-3523 ext. 1305 or Fax information to Kayla Davis at 740-824-5341 or email

Step 4:   Attend the screening session with your child.

Kindergarten screening will be tentatively scheduled during the on Mondays during the months of April and May.  Please note that these dates are subject to change in this ever-changing COVID-19 environment.  The secretary will call and schedule your child's appointment for screening at the respective school.

Conesville Elementary:       April 26 and May 3, 2021
Keene Elementary May 10 and 17, 2021
Warsaw Elementary:  April 12 and 19, 2021

Medical, Dental, and Eye Exams:

Please have the medical and dental forms completed prior to the start of school in August.  Although they are NOT REQUIRED, it is in your child's best interest to have a physical exam and to visit the dentist.  Yearly medical check-ups can monitor your child's development and detect health problems early.  Also, it is HIGHLY RECOMMENDED that children have routine cleaning of their teeth every 6 months to help prevent cavities and other dental problems.

In addition, it is highly recommended, although NOT REQUIRED, that ALL children have a complete eye examination by an eye specialist prior to beginning school to detect vision problems.  Children may not be able to tell their parents if they are having a problem because they do not know what they are supposed to be seeing.

We are looking forward to meeting you and your new kindergarten student.  If you have any questions, please give us a call.  If you know of someone who has a child who will be five by August 1, please call the school office.
